Waiver

I, the undersigned participant (or parent/guardian if under 18), understand that martial arts activities involve physical exertion and contact, and carry a risk of injury.
By signing this waiver, I acknowledge and agree to the following:

Parental Consent (if under 18) – As parent/guardian, I give permission for my child to participate and accept full responsibility on their behalf.

Assumption of Risk – I voluntarily participate in this event at my own risk, fully aware of the potential hazards, including but not limited to sprains, strains, bruises, fractures, or other injuries.

Release of Liability – I release and hold harmless Aikido of Fairfax, Aikido of Arlington, their owners, and instructors, as well as the venue, from any and all claims, demands, or liability for injuries, damages, or losses that may result from participation, except in cases of gross negligence or willful misconduct.

Medical Treatment – I consent to receive basic first aid or emergency medical treatment if necessary and understand that I am responsible for any related costs.

Fitness to Participate – I certify that I am physically fit to participate in this activity and have disclosed any relevant medical conditions.
